Does anyone have a template for the origami dinner menu from the DIY projects on Dec 13, 2007? I really want to do these on my wedding!

I really want to do these for my dinner menu, but would love it if the bride that did them could provide a template. I have Adobe Photoshop and Illustrator and could probably use the template in one of those programs. Also, if she has instructions on how to fold them! Thank you!

These are some really easy to follow instructions. You could probably create a template based on an unfolded fortune you have created
